Registration Requirements
School Year 2010 – 2011
ALL PARENTS MUST PROVIDE PROOF OF RESIDENCY EACH YEAR

Identification

  • Student’s social security card
  • Student’s birth certificate (long form)
  • Parent’s/Legal guardian’s photo identification

Proof of Income

Must Provide One:

  • Last pay stub with name and current address or  
  • Current proof of TANF, SSI, Medicaid

OR

Must Provide Two:

  • Current motor vehicles registration
  • Current lease or rental agreement with recent receipts for payment
  • Current gas, electric, or water bill with payment receipt
  • Current driver’s license or non-driver’s ID

 

Proof of Residency (If not documented with income)


Must Provide Two:

  • Current driver’s license or non-driver’s ID

Health

  • Current physical exam
  • Current dental exam
  • Current health certificate with complete immunization record
