Registration Requirements
School Year 2010 – 2011
ALL PARENTS MUST PROVIDE PROOF OF RESIDENCY EACH YEAR
Preschool – 3 yrs old by September 30th
Prekindergarten – 4 yrs old by September 30th
Prekindergarten – 4 yrs old by September 30th
Identification
- Student’s social security card
- Student’s birth certificate (long form)
- Parent’s/legal guardian’s photo identification
Proof of Income
- Last pay stub with name and current address
- Current proof of TANF or SSI
- Other current proof of income
Proof of Residency (If not documented with income)
Must Provide Two:
- Current motor vehicles registration
- Current lease or rental agreement with recent receipts for payment
- Current gas, electric, or water bill with payment receipt
- Current driver’s license or non-driver’s ID
Health
- Current physical exam
- Current dental exam
- Current health certificate with complete immunization record
